Alaska Commons Surgery Center, LLC seeks to co-locate a single operating room ambulatory surgery center with Alaska Eye Surgery and Laser Center, Inc. which occupies a free standing building located at 235 E. 8th Avenue, Anchorage, Alaska. The estimated cost of the project is $871,108. Alaska Commons Surgery Center, LLC will be operating a maximum of one day per week, and Alaska Eye Surgery and Laser Center, Inc. will be operating a maximum of no more than 6 days per week.

In accordance with A.S. 18.07 and 7 AAC 07, the project does not require a certificate of need because the cost estimate for the leased space does not exceed the $1.5 million statutory threshold under AS 18.07.031.
