Project will establish an outpatient sonogram facility. Total project cost is estimated not to exceed $660,000 with $140,00 for an office lease, $140,000 TI for office space, $200,000 for ultrasound equipment, $60,000 for office furnishing, and $120,000 for computer hardware and software.
Notes:
On October 10, 2007 the Commissioner received a request for determination. On October 18, 2007 the Commissioner requested additional information. A letter from Advanced Sonograms of Alaska, Inc was received in the Commissioner's office on February 28, 2008. Project start date was June 21, 2007 and completed October 6, 2007. On July 11, 2008 Acting Commissioner Hogan determined that a CON was not required for this project.
